Value in Developing Alternative Care Models
Developing new delivery models that embrace alternative payment models goes beyond simply supplying patients with information at time of checkout or discharge. It includes a commitment to patient-centered care, providing clear and useful information to patients, helping patients establish goals and plans to live a healthier life, creating a team of clinicians and administrative staff with clearly understood roles and responsibilities, and using comprehensive systems to support follow-up and tracking of patients. As healthcare moves increasingly outside the four walls of the hospital, it is critical that healthcare executives are well-versed in the opportunities and challenges with alternative care delivery models. Successfully Leading Change
The healthcare environment is constantly changing, and provider organizations must adapt in order to survive and thrive. Organizations are frequently asked to respond to legislative changes, market fluctuations, safety concerns, and patient or staff demands. The success of major change implementation hinges on how it communicated and how it impacts individuals and processes in the organization. Stakeholders frequently resist change for many reasons, including not having enough information, fear of how the change will affect them, comfort with the status quo, and not understanding the benefits of the change. For change to truly be effective, a leader must know how to effectively communicate the benefits, minimize the barriers, and describe the processes.
